No Applications are needed at Distribution Sites
Proof of Residence and Age is required. You may only go to one site. Only 1 set of vouchers is allowed per person per year
Proxy forms must be picked up prior to going to the distribution site.
**Proxy Forms will not be available at the Distribution site.
Each eligible senior will receive one set of 5 – $10 SFMNP checks
